If you are bringing a spouse or any family members, every family member needs to individually complete the form below.

Meet at New Life Church at 6:30PM to help with the meal or provide transportation to and from the event location.

Wearing of face masks is required while traveling to and from event.  Masks are NOT required while eating.

There will be a brief spiritual talk during the event.  We will have a slideshow highlighting events celebrating this past year
